HealthGrid Integrates with Microsoft Dynamics 365 to Automate Patient Interactions During Care Journey

 Next generation CRM platform improves patient acquisition, retention, care plan management

ORLANDO, FL – HealthGrid, a mobile patient engagement leader, today announced the integration of its mobile patient engagement platform with Microsoft Dynamics 365. The integration to Dynamics 365 enables health systems and providers using HealthGrid's patient engagement technology to automate interactions during the patient journey. As a result, health systems and providers can connect with patients at anytime and anywhere, and on any device to deliver specific care communications.

As each interaction occurs, HealthGrid captures specific patient feedback and pushes that data back into Dynamics 365, thus helping health systems and providers uncover rich and actionable data on the patient experience to improve:

  • Patient acquisition and retention – Identify patient populations' care gaps and when to conduct outreach to patients for those required services. The technology also ensures that patients can move to the required downstream providers in accordance with care plans.
  • Post-care to post-acute care plan management – With Dynamics 365, users can identify and enroll patients into HealthGrid's care plan management module allowing for better patient monitoring, including care plan follow up with physicians, compliance to medication therapies, and patient assessments.
  • Patient Relationship Campaigns – Health systems and providers can now boost the effectiveness of their educational and disease management programs, more easily enroll patients into these programs, and deliver individually tailored content via HealthGrid's mobile patient engagement platform.
