Introducing HealthGrid: First ONC Certified Mobile Collaboration Platform for Connecting Care Teams and Patients

ORLANDO, FL – Seeking to solve one of the most difficult challenges in healthcare IT, experienced IT entrepreneurs Raj and Chakri Toleti have founded HealthGrid, a new company and product that vastly improves the communication of patient data to clinicians and patients throughout the care continuum. As a fully-integrated, secure mobile collaboration platform, HealthGrid simultaneously satisfies many CMS Meaningful Use Phase 2 (MU2) requirements by securely transmitting summary of care information between providers; it also gives patients and families access to discharge instructions on any mobile device.

HealthGrid is an end-to-end solution that makes critical patient information accessible and actionable at the point of need. The ONC-certified product is comprised of two services, each of which delivers only the data necessary for the specified purpose:

CareNotify™: A device-agnostic mobile solution that updates patient families with real-time information about their loved one's important clinical events. Patient schedules, medication lists, nutritional updates and relevant educational content are made available automatically, yet without disrupting existing clinician workflows. In conjunction, patients are given access to their care summaries, thus helping providers achieve Meaningful Use requirements.

CareNarrative™: Provides a snapshot view of a patient's clinical state and allows providers to communicate with each other about patient care. CareNarrative effectively delivers patient summary of care records during referrals and/or transitions of care; moreover, it satisfies MU2 mandates while supporting the dynamic creation and management of hybrid, DIRECT and secure non-DIRECT accounts. The integrated task management, notification and note creation features in CareNarrative allow care teams to access critical information across the care continuum.

Proven Leaders
HealthGrid's founders are veteran healthcare IT entrepreneurs. Raj Toleti is founder, and CTO of Coordinated Care Solutions at PatientPoint™, a provider of health information and engagement programs for patients and physicians. Previously he founded Galvanon with Chakri Toleti, a company eventually acquired by NCR, and Cytura Corporation, a content management company acquired by Mobius. Raj Toleti also was a founding member of the Microsoft Consulting Services (Healthcare) practice. Chakri Toleti, Founder and CEO of HealthGrid, also serves as CEO of Mahathi Software Pvt. He previously served as president of both Galvanon and Cytura.
